National Programme: Enhancing and Enabling Quality of Care with the Person with Dementia in the Acute Hospital Setting

Brief description

The programme is designed specifically for acute hospital settings. It aims to raise awareness and understanding of dementia and enabling (and disabling) hospital environments. The programme includesdifferent approaches to supporting the person with dementia in the acute hospital setting including person centred communication, supported decision-making and early identification and management of delirium. It includes dementia/delirium pathways and care bundles to support a person with their dementia journey in acute care.
